Honeywell HomMed

Salem Area Visiting Nurse Association is excited to bring a state-of-the-art monitoring device system to our patients.  With the help of Honeywell HomMed Monitoring system, our nurses can monitor the health status of our home care patients seven days a week in the privacy of their own home.
               
Once all the data is collected, the system transmits the data to a trained clinician at Salem Area VNA for review.  

The clinician uses this information for early detection, intervention, and to educate patients on ways to improve their health.  If any data is out of the pre-set range, or if the user fails to respond, it will be flagged and a call will be made to the patient.  If necessary, a nurse will make a visit to better assess the situation.  

Tele-health monitoring is a great way to catch a problem early and prevent a more serious one from developing.   Physicians whose patients are on monitors can receive reports, including graphs of the data collected, showing their patient’s progress.  Patients who use the Honeywell HomMed monitoring system usually have less visits to the emergency room and have a sense of security in their home knowing someone is monitoring them 7 days a week.
